Health and Wellbeing Service
Our service helps people to become more active, improve their health and wellbeing, and stay independent as they age.
What is the Health and Wellbeing Service?
The Health and Wellbeing Service helps people to become more active, improve their health and wellbeing, and stay independent as they age. Through personalised health assessments and tailored activity programmes, we support people to make positive, lasting changes that improve their long-term health and quality of life.
Our programmes and services
Movement for Health
Movement for Health is a 12-week physical activity programme for people who may be at increased risk of developing heart and circulatory disease. This could be because you have high blood pressure, high cholesterol, prediabetes or other factors that may affect your heart health.
The programme is designed to help you become more active, improve your health and reduce your risk of developing cardiovascular disease in the future. Your GP can refer you to the programme, and we'll work with you to create an activity plan tailored to your health, individual needs and goals.
Visit Movement for Health to find out more and how to access the programme.
Active Ageing
Active Ageing supports adults aged 60 and over to stay active, maintain strength and balance, and continue living independently.
We offer two Active Ageing programmes, depending on how much support you need.
- Our Guided Support programme is for people who would benefit from regular, supervised activity and additional support to improve their strength, balance, mobility and confidence.
- Our Independent Support programme is for people who need less support and feel ready to exercise more independently.
Visit Active Ageing to find out more and how to access the programme.
Community Wellness Checks
Community Wellness Checks are available for anyone aged 18 and over. They're a quick and simple way to learn more about your health, understand some of your key health measurements and identify positive changes that could support a healthier lifestyle - from blood pressure and heart age assessments to BMI and body fat percentages.
Visit Community Wellness Checks to find out more and how to get one.
